dementia nursing service
The dementia nursing service aims to assist people with cognitive decline and/or dementia to remain living independently at home.
We focus on wellness, reablement, and restorative care approaches and support processes for advanced care planning. The service works as part of a multidisciplinary team which includes specialist medical and allied health services providing coordinated care.

The Dementia Nursing Service provides:

  • Comprehensive assessment of physical, social and emotional wellbeing.
  • A team working with you to develop a care plan with a focus on strengths, independence and interests.
  • Assistance and support to link to a range of services.
  • Transition to other services and supports in your community.
  • Ongoing monitoring depending on need.

Services for carers include:

  • Advice on managing behavioural and psychological symptoms of dementia.
  • Advice on managing continence, safety around the home, community and general health.
  • Education through individual and group sessions.
  • Referral to other services as required.

Who can use the service?

  • People who have cognitive decline and/or dementia
  • People who are eligible for a Commonwealth Home Support Programme (CHSP). For more information, visit the My Aged Care website: www.myagedcare.gov.au/help-at-home/commonwealth-home-support-programme
  • People living in the City of Kingston (out of area referrals will be considered on a case-by-case basis)
